Forms
When you schedule your first appointment, find out what forms will be needed for your first visit. In general, if you are scheduling an appointment for NAET, you will need Terms of Acceptance and Patient Agreement, HIPAA form, and History Form A (a total of six pages). If you are scheduling an appointment for chiropractic adjustments, you will need Terms of Acceptance and Patient Agreement, HIPAA form, and History Form B (a total of 8 pages). Please download the forms, fill them out, and bring them with you at the time of your first visit. If you cannot, please arrive at the office at least 20 minutes before the time of your appointment so you can fill them out here. Thank you!
